If you haven’t already completed your new patient forms, you’ll be asked to do so when you arrive. Once we have your personal and insurance information on record, you’ll be taken back to our treatment area. At this point, we will take some X-rays of your mouth and conduct a thorough examination of the teeth and gums before cleaning your entire mouth. We’ll schedule the necessary follow-up appointments based on what we found during the checkup.

Completing the paperwork now could save you 10 or 15 minutes of sitting in our waiting room fumbling with your insurance card and other personal details. We are happy to accept many different PPO dental insurance plans. We are in-network with many different insurance carriers, but we still file claims on behalf of our patients even when they have an out-of-network plan.
